We are excited to be hiring another Shared Living Program Coordinator for our program!




As the Shared Living Program Coordinator, you will be assisting adults with a developmental disability and their families/networks to find long-term, mutually satisfying living arrangements that focus on relationships, personal growth, independence, and participation in the community. This includes ensuring the quality of current shared living settings, the satisfaction of persons served and their support networks, as well as overseeing the recruitment, screening, selection, monitoring, and evaluation of all independent contractors. The Shared Living Coordinator will also be responsible for promoting new and creative individualized options.


You will lead and support practice development both within the team and with providers and will provide coaching and mentoring to staff and providers alike. In order to ensure that the Coordinator has the opportunity to establish and maintain direct involvement with individuals receiving Shared Living services, you will also be responsible for maintaining a caseload. You will ensure that good joint working practices are established and maintained with other internal and external groups.


The role will function as an integral member of the services team, working with personnel across Adult Services and Family and Children’s Services to guide the growth and development of the program to respond to residential service requests, identified needs in the community, and transition planning with adults and youth turning 19.


The successful candidate will have excellent relationship-building and networking skills, a person-centred planning approach, be able to work independently, and possess the written and verbal communication skills to liaise with a variety of stakeholders, including people receiving services, family members, shared living providers and funders.

In the role of Shared Living Program Coordinator, you will be on the go, going between meetings with providers and people served. Helping with the onboarding of new providers, and meeting with extended support networks of the folks we support.   • Degree in related Human/Social Service field, or equivalent combination of education and experience.
  • Minimum of five years recent related experience
  • Case management and Service Coordination experience
  • Excellent time management and organizational skills
  • Strong written and verbal communication skills
  • Strong computer skills
  • Knowledge of current theory, principles and practice in the field
  • Satisfactory Criminal Record Check
  • CPR/First Aid Certification
  • Class 5 Driver’s license and use of personal vehicle
