North York Seniors Centre provides programs and services to promote the physical, emotional and social

well-being of adults 55+ living in our diverse community.

 

This position reports to the Supervisor, Hesperus Care

Responsibilities:

  • Provide program facilitation to clients admitted into the Hesperus A Day Away Adult Day Program 
  • Plan monthly activities that are goal oriented and design monthly program calendar
  • Provide support to the Sheppard A Day Away program design and facilitation 
  • Contribute to Care Plans development for clients, based on mutually agreed upon client goals
  • Support transitions in care, facilitating connections with internal Care Navigators and external resources as needed
  • Actively participate in continuous quality improvement processes and assist the Team with research and development of best practices
  • Promote and maintain a high standard of customer service that is client focused

 

Qualifications/ Skills:

      Education:                  

  • College diploma in Therapeutic Recreation Assistant or Activation

      Experience:                

  • Working with frail and cognitively impaired seniors/adults
  • Adult Day Program experience preferred

        Other:                           

  • Valid driver license and access to a vehicle
  • Strong interpersonal and organizational skills
  • Computer skills in a Windows environment; knowledge of Goldcare, an asset
  • Member of Therapeutic Recreation Ontario, an asset
  • Fluent in verbal and written English; other languages, such as Cantonese and Mandarin, an asset
  • Current First Aid and CPR certification
  • Recent Vulnerable Police Check
  • Compliant to NYSC’s Vaccination Policy
