Francophone Health Care Navigator 2022 Job Description


Black Creek CHC is a community-based primary care organization that is committed to addressing the social determinants of health in a client-centered, equity driven manner.  Our inter-disciplinary team of care providers provide comprehensive primary health care to registered clients of our Centre.  The Francophone Health Care Navigator will also be responsible for networking with Francophone partners within Central LHIN to advocate for this client population. 

 

Job Summary

The Francophone Healthcare Navigator (FHCN) will facilitate access to care and care coordination for French Speaking clients and families. The FHCN will facilitate linkages between primary care, allied health and social services to ensure smooth transitions across the health care system for clients and their caregivers.  The FHCN will be responsible to develop client-centered pathways that are linguistically and culturally relevant to the Francophone population, ultimately assisting clients to move smoothly through the healthcare system.  The FHCN will also be responsible for data collection and analysis, and assist in the development of quality and process improvement initiatives.

 

Key Responsibilities:

  • In collaboration with the bilingual Community Health Worker and other healthcare professionals, outreach to Francophones in North York
  • Design and implement health promotion services and programs for Francophones
  • Support the Francophone advisory committee comprised of Entité 4 and Francophone serving agencies across Central LHIN
  • Act as non-clinical liaison/navigator for clients in all aspects of their care journey
  • Facilitate client appointments within the health and social service sector
  • Complete timely documentation, utilizing identified electronic record software
  • Ensure clear and timely communication between primary care provider and allied services
  • Assist and empower clients and caregivers to engage in self-management of chronic conditions by establishing relationships, follow up, and providing resources as needed
  • Identification, management, and compilation of data metrics to measure impact
  • Participate in case conferences and team planning meetings
  • Participate in program committees as assigned
  • Handle other duties as assigned

Qualifications

  • Master’s Degree in Health or Social Sciences field with a minimum of three years’ work experience in the Health and/or Social Sciences field
  • Fluency and proficiency in written and oral French
  • Experience working with, and extensive knowledge of the local health care system and community resources related to the client population
  • Experience working in culturally, socially and economically diverse communities
  • Excellent interpersonal, oral and written communication skills
  • Proven ability to work as a team member
  • Demonstrated ability to exercise sound judgment and decision-making skills
  • Experience with quality improvement initiatives related to care delivery processes
  • Demonstrated initiative and experience working independently with minimal supervision
  • Proficiency with Microsoft Office programs, internet research, EMR (PSS) and ability to learn and master other computer programs as needed
  • Flexibility to work evenings, weekends and at various locations
