Job Description
Position: Registered Nurse – Certified Practice, RN(C)
Experience/Specialty: 5 years experience in a variety of settings.

Position Summary
The primary care nurse cares for families and individuals across the lifespan. This includes health promotion, screening and assessment, counselling and education, and treatment and for minor and acute illnesses, and chronic care management within the nursing scope of practice. The primary care nurse is expected to be able to work independently and work within the standards, limits, and conditions determined by relevant legislation and BCCNM; and the limits of the nurse's individual expertise. The nurse is expected to consult with a doctor when appropriate. It is important that the primary care nurse be willing and interested in working collaboratively as part of the interdisciplinary team at REACH; be committed to the goal of improving the health of people living in the community; and adhere to the REACH code of ethics. The nurse reports to the operations manager and is to follow protocols and  procedures as set out by the operations manager. The nurse will always act within scope of practice as set out by the BCCNM.


Responsibilities
• With BCCNM certification assess, diagnose and treat sexual transmitted infections and provide contraceptive management as per BCCNM guidelines and relevant decision support tools.
• With BCCNM certification, assessment, diagnosis and treatment of specific acute and minor illnesses in adults and children in accordance with BCCNM guidelines and decision  support tools within individual scope of practice.
• With BCCNM certification, contraception counseling, treatment and follow-up.
• Well-child care including assessment of growth and development, and family function.
• Preventative care including: cancer screening, chronic disease management, harm reduction services.
• Perinatal care, including chest/breast feeding support, prenatal assessment and support, postpartum assessment and support, pregnancy testing and, pregnancy counseling (to include referral to abortion services if requested).
• Prioritization, case-management and scheduling of walk-in patients during drop-in hours as the Nurse of the Day (NOD).
• Limited supportive counselling for patients in crisis; referrals as needed to appropriate community resources.
• Screening for diseases such as diabetes, hypertension, addiction.
• Preventative care such as adult and childhood immunizations, screening and treatment for STIs, basic nutritional counseling and smoking cessation, support for healthy lifestyle modifications, and assessment of readiness for change, breast and colon cancer screening.
• Chronic disease management, in conjunction with the team.
• Advocacy for patients as needed.
• Referral to community programs and/or support services as needed.
• Consult with a doctor at any point in the patient’s care when the nurse approaches his/her limits of scope of practice or competence.
• Triage patient phone calls.
• Complete chart and/or electronic medical records.
• Teach nursing or medical trainees.
• Keep clinic stats as needed
• Participate in program development and regular professional development
Team and centre activities:
• Participates actively in staff meetings and board committees.
• Participates in the overall activities of the health centre, e.g. in service training, committees, special events and shared staff responsibilities.
• Communicates clearly, listens accurately, is open to feedback, handles conflict appropriately, and displays sensitivity to others.
• Collaborates well with others, promotes cooperation and teamwork.
• Participates in REACH’s commitment to becoming discrimination free and an inclusive health centre and promotes a healthy lifestyle.
• Maintains the reputation and confidentiality of the health centre and its clients at all times.
• Adheres to all applicable REACH policies and procedures.


Qualifications and Skills
• Completion of a recognized Registered Nurse education program and current registration to practice in BC.
• BCCNM – certified in Contraceptive Management and STI-Management.
• Remote Certified Practice is an asset
• BCCNM certified practice with experience working in an independent practice.
• Significant nursing experience (minimum five years) in a variety of settings.
• Current HCP First Aid Certification.
• Experience in Public and/or Community Health Nursing.
• Adult and child immunization competency as per BCCDC.
• Experience with mental health and addictions an asset.
• Ability to communicate effectively with patients, co-workers and community.
• Ability to work in a respectful way with a wide range of patients.
• Experience working with marginalized populations (mental health and addictions, poverty, vulnerable youth, LGBTQ2I+, and other community members)
• Ability to work both independently and collaboratively within an interdisciplinary team.
• Commitment to the concept of the Community Health Centre as an effective means of achieving comprehensive health care.
• Ability to work in a respectful way with a wide range of patients.
• Ability to speak a second language is an asset
