2022 Job Description


Black Creek Community Health Centre is a community based non-profit organization that provides primary health care and health promotion programs and services to community residents. BCCHC is seeking a Registered Dietitian to join our multi-disciplinary team. If you are interested in addressing individual, group and community health issues while working in a supportive environment and maintaining a positive work-life balance, you will be interested in joining our team. This position requires flexibility to work from any of our various service delivery sites including our two main locations at Sheridan Mall and Yorkgate Mall, and a 3rd location (Bathurst / Sheppard). Please note that working on Saturday is a requirement for this position.





The Dietitian plans and directs the nutritional care of clients and participates in community health education (groups and individuals) to promote health through better nutrition.




  • Perform nutritional assessments, develop client-focused nutritional plans, and provide health education/counselling and other clinical activities.
  • Develop and/or obtain nutritional education resources to meet the needs of the clients and the Centre.
  • Ensure appropriate, comprehensive treatment is delivered by maintaining client records and encountering client data, and by answering inquiries and directing clients or providing appropriate treatment within the safeguard of confidentiality.
  • Work with Centre staff to develop, implement and evaluate nutrition health promotion activities and new programs in the community by identifying health needs and increasing awareness of nutrition services.
  • Assess community nutrition needs and barriers to acquiring adequate nutrition as part of our food security initiative.
  • Participate in team meetings and centre-wide community initiatives to improve access to adequate nutrition.
  • Maintain and enhance professional competencies through appropriate continuing education.
  • Assist in developing and revising protocols, procedures and policies as required by the Centre to improve client services.
  • Handle other duties as assigned by their supervisor or designate.







  • Extensive knowledge of and proficiency in nutrition care.
  • Excellent clinical assessment skills.
  • Experience in clinical counseling, community nutrition, harm reduction and food insecurity
  • Training in Infection Prevention and Control (IPAC) an asset
  • Familiarity with scheduling and electronic health record keeping, preferable with Practice Solution Suite (PS Suite)
  • Demonstrated ability to work effectively in a multi-disciplinary team environment.
  • Commitment to and knowledge of community-based health care and resources.
  • Excellent communication and interpersonal skills.
  • Proficiency in the use of computers and various software applications.
  • University degree in food and nutrition from a recognized university.
  • Completion of an accredited Dietitian Internship.
  • Membership with the College of Dietitians of Ontario.
  • Knowledge and sensitivity to the impact of social, economic, structural, environmental, and cultural issues on health.
  • Proficiency in a second language relevant to the community is an asset.
  • Able to work a flexible schedule (evenings, weekends) and at different locations.
