DESCRIPTION
One .5 Permanent Position (17.5 hours per week)
This position will provide intensive case management and advocacy services to individual’s age sixteen and over who are experiencing mental health, dual diagnosis and/or addictions and who are vulnerably housed or homeless. Through a collaborative approach, this position will facilitate recovery and assist service users who are experiencing significant difficulties in daily functioning to improve their quality of life. Tasks will include direct service interventions, clinical support and community development. Daily travel within York region will be required.
About the Organization:
York Support Services Network (YSSN) is a non-profit agency providing streamlined access for mental health and addictions services as well as case management and community crisis response services within York Region and South Simcoe. It is a designated administrator of the Developmental Services Ontario program and offers access for adults to developmental services in central east region.
