As the Director of Mental Health Case Management,  you will work with our team to provide services and support to meet Harvest Project’s organizational mandate. Your experience and expert knowledge of assessment and case management best practices will create innovative solutions and programs,  provide oversight, and direct the effective delivery of Harvest Project’s integrated case management model. You will provide expert opinion and direction to the organization on all aspects of case management best practice, including conducting assessments and intakes, program design, and compliance with all relevant legislation.

 

About You:

  • You have completed a Masters' program in the field of social work from an accredited post-secondary institution, with experience to practice social work with advanced competency.
  • Your clinical orientation and training allows you to make appropriate mental health assessments in order to manage cases effectively.
  • You are a member in good standing of a provincial / Canadian professional association, meeting all ongoing mandatory professional development requirements.
  • You have demonstrated clinical experience with conducting assessments, program planning, implementation, and evaluation.
  • You demonstrate compliance with all ethical standards to keep clients safe and maintain the highest quality of service and privacy.
  • You have a sound knowledge of the policies, procedures, laws, and regulations that govern the case management process for serving vulnerable adults and their families.
  • You have a minimum of five year’s experience working in case management, with an expert knowledge of case management best practices to support successful client outcomes.
  • You have demonstrated previous experience in program design and implementation.
  • Your strong personal leadership skills focus on training, mentoring, coaching, and collaborating, to create effective teams.
  • You possess the ability to de-escalate challenging situations using advanced conflict resolution skills, compassion, and empathy.
  • You are able to appropriately manage work stresses in order to support personal mental health.
  • You are organized, detail oriented, deadline driven and able to prioritize to meet organizational goals.
  • You are proficient with MS Office and data bases.

 

Additional Information

  • Signatory to, or acknowledgement of, Harvest Project Statement of Faith is required
  • At times may come into contact with clients in crisis situations
  • Signature and adherence to Confidentiality Agreement
  • Requires a criminal record check and vulnerable sector search

 

Commitment

37.5 hours/week:  7.5 hours per day, 5 days per week: Monday to Friday 9:00-5:00; some flexibility by arrangement
