Primary Health Care Coordinator
Black Creek Community Health Centre (BCCHC) is a community based non-profit organization that provides primary care and health promotion programs and services to residents within its catchment area. In response to increased demands on BCCHC in providing services within Northwest Toronto, this new role will support care delivery at all BCCHC’s service sites. Reporting to the Senior Director of Primary Health Care (PHC), the Coordinator will ensure smooth and efficient service delivery at all BCCHC sites, including providing oversight of front desk reception functions, partnership support and operations.
Responsibilities:
- Work with management to coordinate operations at all BCCHC locations and outreach sites
- Supervise reception staff, including managing schedules for coverage at all sites
- Ensure all sites have adequate equipment, furniture ad supplies to support service delivery
- Coordinate use of space at each location, including booking of shared space
- Observe recurring reception areas issues and work with team to identify training to optimize client/customer service
- Update staff of new programs and schedules at each site and provide direction on how to share information with clients
- Coordinate workflow for reception staff and promote a collaborative environment across all BCCHC programs and services
- In collaboration with HR, coordinate orientation for new reception staff
- Provide operational support to staff at various sites
- Coordinate meetings with external partners as they pertain to work at the different sites
- Ensure sites meet Health and Safety and Privacy compliance standards
- Oversee and coordinate vendors related to Primary Care service delivery: Labs, On-call schedules, Answering Service, etc.
- Participate in Non-Insured CHC working group
- Other related duties as required
Qualifications:
- Medical Administration Certificate and/or equivalent with relevant experience
- At least ten years experience in a community-based healthcare setting
- Thorough knowledge of BCCHC programs and services across all service sites
- Experience supporting primary health care service delivery in community
- Strong administration and coordination skills with experience in supervision/teamwork
- Excellent customer service, and communication skills that demonstrate empathy and compassion.
- Able to function independently and maintain a high level of confidentiality
- Knowledge of issues affecting Northwest Toronto residents and similar communities facing access to services
- Proficiency in a language spoken by priority population is an asset
- Experience working in a diverse setting, with commitment to anti-racism and anti-oppression
- Understanding of the CHC model of care and the Social Determinants of Health
- Proficient in PC (Microsoft Word, Excel, Outlook) & medical software (Telus PSS)
- Ability and flexibility to work at multiple locations and evenings or weekends

Salary: $62,000 - $70,000
