Carya is searching for a Link Worker to work in collaboration with Aging in Community Support Outreach Workers to assist participants in achieving their service goals. Under the direction of the Supervisor, the Link Worker will utilize best practices in social prescribing to facilitate the connection of participants with various social, community, and partner supports. This will enable participants to enhance their health and well-being, assisting them in aging safely in community.
The Aging in Community Supports (AICS) program aims to improve the integration of health and community-based older adult services. The focus of the program is on improving coordination of non-health related services that support healthy aging in community and enhancing the quality of life for older adults.
