Nurse Practitioner – UHN Social Medicine Program

 

2 Full Time positions (1.0 FTE; 35 hours/week),

University Health Network (UHN) is committed to high-quality and equitable care for all patients. Furthermore, UHN recognizes the significant health gaps that continue to exist for patients with unmet health and social needs, including housing, food insecurity, and poverty. To advocate for these patients, UHN established the Gattuso Center for Social Medicine to transform how we deliver care for marginalized populations.

 

General Responsibilities:

 

This is a new partnership initiative between the Centre and UHN to focus on caring for patients who are high utilizers of UHN services that can be better served outside of the ED. To meet this objective, this partnership is hiring Nurse Practitioners to address the social determinants of health for patients with unmet complex medical and social needs. The Nurse Practitioners will work closely with the Social Medicine Community Health Worker and provide support within the hospital and mobile services in the community.

 

This is a seconded position reporting to the Senior Manager, Program Operations, Social Medicine Program at UHN. The successful incumbents will be responsible for providing care that optimizes patient outcomes, enhances safety, effectiveness, and access to appropriate health care for these clients. The Nurse Practitioner will collaborate with other nurse leaders, physicians, and inter-professional teams both at UHN, and other community organizations as required to develop, implement, and evaluate best practices to deliver holistic care.

 

Collaborating with UHN clinicians, members of the Social Medicine team and community health workers, and other community staff, the Nurse Practitioner will develop comprehensive plans to help address individual clients’ social and complex medical needs. The incumbents will be expected to perform cross-functional and cross-organizational duties consistent with the role of a Nurse Practitioner and perform other relevant duties as assigned.

 

Skills & Qualifications:

 

  • Successful completion a master’s degree Program in Nursing and CNO recognized Nurse Practitioner program
  • Current registration as a Registered Nurse in the Extended Class in good standing with the College of Nurses of Ontario (CNO)
  • Membership in the Registered Nurses Association of Ontario (RNAO) and/ or other relevant professional organization(s)
  • 3 to 5 years nurse practitioner experience in mental health, addictions, harm reduction and/or primary care setting
  • Experience cultivating partnerships with community organizations as well as social and health resources
  • Experience/knowledge of partnering with marginalized/socially vulnerable populations, providing trauma informed care and related advocacy work
  • Demonstrated excellent verbal and written communication skills
  • Highly effective interpersonal and client service skills with demonstrated history of effective decision making skills
  • High level of comfort with diverse populations; demonstrated ability and comfort in working with people who use drugs and people with mental health challenges
  • Able to work independently and as a team member within a multi-disciplinary environment
  • Cross-appointment and/ or eligibility to an academic institution and experience in academic esearch preferred

 

Compensation: $102,652 to $122,178 per annum plus Group Benefits and HOOPP (Healthcare of Ontario Pension Plan)
