Intake Worker Contract Position - One Year

The Developmental Services Case Manager – Intake is responsible for the provision of case management and advocacy services to children and adults who have a developmental disability or dual diagnosis (developmental disability and mental health). The main functions include short term case management, intake functions such as needs assessments using specific tools, eligibility screening for children’s referrals, warm transfers/referral support and community education.

 POSITION REQUIREMENTS

• Bachelor’s degree in Social Work or related discipline (i.e. Health Sciences, etc.)

• Three years directly related experience

• Experience working with individuals who have developmental disabilities is essential

• Language skills and an understanding of cultural diversity are considered assets

• Familiarity with the local service system and its providers is required

• A second language is considered asset

• The successful applicant will be required to provide a police clearance that meets agency standards

• A valid license, appropriate business use insurance and regular use of a reliable vehicle
