Program Facilitator

The incumbent for this position will have a Bachelor of Social Work Degree from an accredited university and demonstrated experience in delivering needs-based, culturally grounded and trauma informed supports for justice involved youth.  They will have lived or learned experiences interacting with vulnerable youth, a strong knowledge of the criminal justice system, familiarity with community-based and criminal justices services in the GTA and specialized knowledge of the challenges that overrepresented and marginalizes populations, including Black, Indigenous and racialized youth experience in the justice system.   

 

The incumbent will have excellent conflict resolution skills, case management and facilitation skills. Familiarity with Cognitive Behavioural Interventions, Motivational Interviewing and Stages of Change and identification of criminogenic factors as they relate to developing individual intervention plans is essential.

 

The focus of work for this position will include responsibility for the development of individual participant goals and plans geared to personal development and opportunities for positive change for program participants. You will also be expected to coach/mentor and provide participants with relevant and evidence-based service interventions to promote assets, competency, and resiliency. You will assume service coordination functions while acting as an advocate and providing information and referrals to program participants. Mentoring and counselling program participants to encourage social inclusion and engaging these program participants to identify issues, needs, goals, aspirations, skills, abilities, and services needed as well as barriers to accessing these services is critical. To carry out brokerage/advocacy/resourcing functions that ensure participants have access to programs, services and opportunities needed for healthy development by building and maintaining relationships with internal and external stakeholders and to demonstrate appropriate behaviours consistent with CTI’s Mission, Vision, and Values.

 

The program is designed to address criminogenic risk factors, improve pro-social attitudes and beliefs, address anti-social thinking, develop skills to foster pro-social change and help build intervention and relapse prevention strategies for the youth.

 

The Program Facilitator will liaise with a variety of community resources and personnel within the criminal justice system, referral sources and community agencies in facilitating achievement of client goals.

 

 

MAJOR DUTIES

 

The Incumbent for this position will be responsible for the following:

  1. Intake and Assessment Interviews, including Individual Planning and Case Management
  2. Maintaining case recording on client interactions as appropriate and as per agency guidelines, leading to overall assessment of client performance and impact of service
  3. Provide regular progress reports on program participants
  4. Maintain and provide data activity and outcome/impact reports
  5. Developing and maintaining a broad base of diverse partnerships, including community partners to assist clients with such issues as housing, income, etc.
  6. Active participation and contribution to the Client Programs Team and its members
  7. Representation of CTI at various community forums/conferences/committees tasked with assessing and responding to community matters involving our client population
  8. Ensuring accuracy, confidentiality and safekeeping of agency records
  9. Participating constructively in the supervision process and exercising due diligence with respect to self-care and accountability for practice
  10. Assigned Other Duties in Accomplishing Goals of the Program

 

Although the majority of the work will take place between 9 a.m. – 5 p.m. Monday to Friday, some evenings and/or weekend work may be required. 

Salary range for this position is $60,000 - $75,000 based on qualifications.  The incumbent is eligible for enrollment in the agency benefits plan.  A valid Ontario driver’s license with access to a reliable vehicle for local travel is required.  A VSS form must be provided.
