The Community Resource & Outcomes Facilitator is responsible for meeting the vision, mission and values of Epic Opportunities while developing and implementing creative strategies that will support the people we serve to gain valued social roles and relationships through connections within their community. The Facilitator will liaise and develop strategic partnerships with community organizations that build inclusive environments for people with developmental disabilities to participate and contribute. Additionally, the Community Resource & Outcomes Facilitator will support the implementation of Personal Outcome Measures ensuring that individuals are consistently supported to pursue and achieve identified goals.  They will help track person centred planning across the organization ensuring that planning frequency meets or exceeds provincial standards.  They will be responsible for providing tools to coach and mentor the teams responsible for creating, achieving, and documenting said goals.

This is a full-time (80 hours biweekly) position.

  • Excellent written and oral communication skills, with the ability to listen, understand the problem and respond to various situations.
  • Network with external professionals and draw expertise from these relationships to inform best practices and meet a quality framework.
  • Continuously engage in organizational improvement to enhance quality of life for all people served.
  • Ability to lead and develop others to assist vulnerable people in achieving personal outcomes.
  • A valid Manitoba Driver’s License and use of a personal vehicle is required.

Qualifications and experience we are looking for:

  • Degree in Social Services or Disability and Community Support Diploma.
  • Previous work experience in human services with a focus in disability supports an asset.
  • Other combinations of education and experience will be considered.

 Required Documentation:

  • Criminal Record Check with Vulnerable Sector Search.
  • Adult Abuse Registry Check.
  • Child Abuse Registry check.
