What you'll be doing:

  • Conducting Person Centered Plan (PCP) interviews with the individual, their support network and guardian (if applicable), in order to determine the individual’s employment aspirations, experiences, abilities, skills and potential obstacles.
  • Monitoring and developing the Individual Service Plan (ISP) documents in collaboration with the individual and his/her supports and stakeholders, based on the individual’s desires, goals, and needs.
  • Meeting with employers and individuals to establish work goals and develops implementation plan to ensure that employment coaches and employer are aware of the individuals’ goals.
  • Breaking job duties and assignments into specific steps to facilitate the individuals learning and independence, ensuring quality of completed tasks.
  • Monitoring individual’s performance including determining when the individual is capable of performing the job independently.
  • Assisting and advising individuals in building natural supports in the workplace.
  • Providing on going assessment of supported individuals, determining necessary changes to meet the support needs of the individual, and providing necessary training and support for successful job performance.
  • Utilizing ETO to maintain accurate and up-to-date individual time records, case notes, assessments, and files for Individuals.
  • Maintaining and updating individual’s files.

 

What you bring to the table:

  • Minimum 2 years of post-secondary education.
  • Minimum 2 years of experience working with individuals with disabilities and/or barriers to employment.
  • Minimum of 2-3 years of case management experience.
  • Must be eligible to work in Canada.
  • First Aid certificate an asset.
  • Own vehicle, a valid class 5 driver’s license, and a clean driving record.
  • Ability to handle sensitive and confidential information appropriately.
  • Interest and ability to work with individuals with disabilities.
  • You deliver amazing service by going above and beyond for all Goodwill stakeholders.
  • You demonstrate teamwork and engage fellow Team Members in contributing to Goodwill’s mission & core purpose.
  • You have an open and honest attitude while making moral decisions.
  • You recognize the great efforts of fellow Team Members through genuine recognition activities.
  • You contribute to a welcoming & accepting work environment by showing respect at all times.
  • You hold yourself and fellow Team Members accountable in their roles and for their contributions to Goodwill’s mission & core purpose. 
  • You continuously look to improve yourself and the organization in any aspect including but not limited to processes, communications, tasks and customer experience.
  • You prioritize safety in all tasks, ensuring safety of self as well as fellow Team Members.
