About the Role

The Caregiver Link Worker is a new position and works exclusively within the Caregiver Rx Social Prescribing Project, working collaboratively with the health and social sector to address a caregiver’s needs within the social determinants of health. Central to the role is the delivery of individualized support (by phone, virtual and in person), to address immediate practical concerns as well as ongoing needs that contribute to a caregiver’s physical, social, and emotional wellbeing. This includes providing emotional support, assistance with navigating health and social services, and connecting caregivers with community resources that address a wide range of needs, such as mental health, financial support, housing, social opportunities, exercise, and nutrition. The Caregiver Link Worker will build ongoing relationships with caregivers, co-create wellness plans, and use motivational interviewing and action planning to help caregivers meet their wellness goals. Reporting to and working closely with the Caregiver Rx Social Prescribing Project Lead, the Caregiver Link Worker will also play an integral role in developing and maintaining referral pathways into our program within the wider health and social care community. 

Key Responsibilities:

  • Contact caregivers in response to incoming referrals: provide empathic 1-1 emotional support, assistance with navigating the health care system, formal social prescribing intake assessment, and/or referral to community resources.
  • Use of pre and post screening tools to assess caregivers’ strengths, emotional capacity and stress levels.
  • Co-create wellness plans during initial and subsequent meetings to address a caregiver’s social determinants of health.
  • Assist with problem solving and creating action plans where appropriate using a Brief Action Planning approach.
  • Assist with accessing services/resources where appropriate.
  • Liaise with community programs and maintain outgoing referral and feedback pathways.
  • In partnership with team members, regularly update community resources.
  • Share insights on caregiver needs to inform the development of tools and resources.
  • Maintain client record data in a timely and accurate manner via the FCBC database (Salesforce-Non-Profit based) 

Qualifications and Experience:

  • Degree in healthcare field, social services, geriatric care, mental health or other related discipline, or a combination of education and experience.
  • 3 years case management experience supporting individuals and/or family caregivers.
  • Experience applying motivational interviewing techniques to support others in meeting their goals.
  • Excellent interpersonal and communication skills (verbal and written), with a demonstrated ability to provide empathic, warm, and professional support that is accepting and inclusive of all clients.
  • Experience working with groups an asset, with facilitation skills, preferably in a workshop environment.
  • Presentation skills an asset.
  • Caregiving experience an asset.
  • Proficient in Microsoft Office: Word, Excel, Outlook and must have a high level of comfort using online web platforms and digital tools (and the ability to simplify processes for technologically unskilled users).
  • Demonstrated ability to work independently and as part of a team.


Location of Work: Victoria, BC at the FCBC office, with the potential to also work from home (hybrid).

Hours of Work: Typically working during our office hours Monday – Friday, 8:30 AM – 4:00 PM and occasionally after-hours for caregiver appointments or other FCBC activities.

Compensation: commensurate with experience in the range of $27 - $31 per hour.
