What We Can Offer You:
Position Profile
The Intense Case Manager (ICM) works collaboratively with the Our Place Peel "Home for Good" team and community organizations to support the most vulnerable and chronically homeless youth, ages 16-24, in the Region of Peel. The Intense Case Manager provides assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual's comprehensive needs in order to achieve greater life stability.
The ICM empowers youth, draws on their own strengths and capabilities, and promotes an improved quality of life by facilitating timely access to the necessary support and thus reducing the risk of homelessness.
This is a full-time position with the flexibility to work evenings and weekends.
