Nurse Practitioner – Hepatitis C

Full-Time Regular Position. 1.0 FTE (35 hours/week),

Bargaining Unit – Ontario Nurses Association (ONA) Region 3 - Local 95

General Responsibilities: We are currently seeking a Nurse Practitioner (NP: RN-EC) to work in our HEP C team. The HEP C NP provides direct HEP C treatment to clients who test positive for Hepatitis-C Virus (HCV).  The NP treats non-acute or non-chronic health care needs of clients within the treatment period timelines.  The NP will work within a harm reduction framework to provide assessment and treatment functions for HEP C clients and work with the broader HEP C team in providing administrative and evaluation support for the program.

 

Specific Responsibilities:

  • Conduct in-depth and focused assessments based on client condition and acuity at intake and on an ongoing basis; provide health and harm reduction education as needed.
  • Perform outreach and client engagement through community partnerships in areas of high HCV infection rates.

•         Perform assessment, diagnosis, treatment, health education/counselling and other clinical activities for Primary Care and Harm Reduction (PCHR) clients.

•         Build and maintain partnerships through outreach and pop-up clinics when/where appropriate.

  • Liaise with other providers including Physicians, Nurse Practitioners, Registered Nurses, and Supervised Consumption site staff to coordinate client care.
  • Engage in collaborative community relationships to facilitate patient access to services.
  • Charting client assessments, procedures, follow up per the College of Nurses of Ontario (CNO) standard.
  • Contribute to chart documentation, participate in chart reviews, case conferences, and make referrals.
  • Case coordination with HEP C team and clients.
  • Contribute to knowledge exchange products and deliverables that are required by the funder.

 

Qualifications:

  • RN (EC) training from a recognized program
  • Current registration as a Registered Nurse (EC) with the College of Nurses of Ontario
  • 3-5 years of experience as a RN(EC)
  • Experience in a community setting or combination of community, hospital, or public health settings
  • Knowledge of harm reduction principles, injection drug use and associated health issues
  • Demonstrated ability to de-escalate conflict
  • Confidence in asserting and maintaining strong boundaries
  • Knowledge of community services and resources; strong advocacy skills
  • Proficiency in the use of electronic medical records
  • Strong oral and written communication skills
  • Respects and values the diversity of communities and individuals

 

Compensation: $115,525 to $137,500 per annum, plus Group Benefits and HOOPP (Health Care of Ontario Pension Plan)
