Job Summary:
The Senior Services Coordinator is responsible for coordinating support services to multi-ethnic seniors in the City of Vancouver. This position is also responsible for supporting seniors with higher needs and bridging them to local culturally-appropriate community resources and programs that promote their healthy aging and well-being. The Senior Services Coordinator works closely with other staff members that are providing senior services, and other key community stakeholders including the local health authority, primary care network, local family doctors, as well as community organizations.
Reports To: Senior Manager
Key Duties and Responsibilities:
- Develops and maintains supportive relationships with existing and new senior clients receiving program services
- Liaises and works with community partners on the project
- Coordinates, recruits and intakes volunteers to support ongoing services
- Maintains service records, collects data, and prepares reports (including service outputs and outcomes as well community service gaps) in accordance with funder and organizational requirements
- Conducts outreach and implements marketing efforts
- Connects with senior who are in need of services and supports
- Conducts needs assessments and develops person-centered wellness plans with clients
- Bridges clients to culturally-appropriate local community resources that promotes healthy aging and well-being through enhancing awareness of available resources, supporting clients to navigate systems, as well as conducting facilitated referrals
- Follows-up with clients to ensure they are connected to appropriate community services and provides additional support as required
- Participates in community meetings and events, communities of practice, training, as well as researches and evaluates activities as needed
- Ensures all services provided meet service standards, are aligned with best practices, and are consistent with the agency’s philosophy
- Supports the implementation of the funder’s learning and quality assurance plan which includes surveys, focus groups, interviews, and other evaluation activities
- Collects feedback from clients and partners, and prepares analyses of findings
- Participates in the division’s performance and quality improvement (PQI) activities
- Other related duties as instructed by the Senior Manager
Education, Training and Experience:
- Bachelor’s degree in Social Sciences, Health, Education, or related discipline; equivalent combination of work experience and education may be considered
- Education in social work is an asset
- A minimum of 2 (two) years’ experience in delivering services in a community-based social service setting, preferably for multi-ethnic seniors
- Experience working with diverse client population groups
- Experience working with senior populations
- Experience in conducting and interpreting needs assessments and working with clients to develop individualized service plans
- Experience in supervising and working with volunteers
- Knowledge of community resources and services for multi-ethnic seniors in the local geographic area
- Knowledge of individual and systemic barriers/challenges that impact the health and well-being of multi-ethnic seniors in Canada
