Applications are currently being accepted for a full-time (35-hours/week) Adult Protective Services (APS) Worker in our Developmental Services program.
Job Purpose
To assess needs, and develop and implement personal Individual Support Plans for adults who have a developmental disability (developmental disability as defined by the Government of Ontario’s Select Committee on Developmental Services) to enhance their ability to continue to live independently in their community. The objective is to support individuals and, if permission is given from the individual, their families or caregivers in continuing independent living and integration into the community.
Job Responsibilities
Provide individualized supports to adults with developmental disabilities (developmental disabilities as defined by the Government of Ontario’s Select Committee on Developmental Services) who are living independently in the community
Provide educational & learning assistance in order that individuals are able to live independently & without endangering themselves & others
Facilitate & coordinate the necessary supports individuals may require &/or seek which they identify for themselves
Support individuals in developing friendships within the community & affiliations which decrease their sense of isolation
Help mediate, facilitate & inform individuals to make choices for themselves & promote self determination
Provide a range of instrumental & counselling (practical & supportive) services to individuals
Provide crisis support & immediate intervention supports
Work with individuals who have complex & challenging needs
Work with other community agencies & community personnel
Maintain current, accurate & detailed files & written support plans, which meet agency & funder requirements
Correspond in a manner which is clear, concise, factual & accurate
Follow all regulated policy & procedure as outlined by Agency, funders & appropriate governing legislation
Receive, assess & support all eligible persons
Participate & represent the Agency in community activities as they may relate to Developmental Services
Comply with the criteria for Serious Occurrence Reporting (SOR)
Facilitate group education & instruction
Establish & maintain positive relationships with key community collaterals
Other duties as assigned
Key Qualifications
Bachelors level degree in Social Work/Psychology/Social Development combined with a minimum of 3-years paid work experience working with adults who are developmentally disabled individuals (developmental disabilities as defined by the Government of Ontario’s Select Committee on Developmental Services)
Demonstrated experience in working with people with a dual diagnosis (complex mental health needs & developmental disability)
Current 1st Aid & CPR certification
As there may be a work-from-home component to this job, the ability to work from home, consisting of a safe and private home office setting and the necessary technical equipment and high-speed Internet access for remote work
Familiarity with virtual work platforms
Ability to mediate conflict
Valid Ontario driver’s license, a clean driving record, appropriate insurance & daily access to a reliable vehicle.
Superior written & verbal command of the English language
Computer proficiency in MS Word, Excel & data bases
Demonstrated efforts to continuously update skills
Additional language(s) an asset
Ability to work days, evenings & rotating Saturdays
