Position Overview:

As a member of the Health Services team, the Manager leads the planning, design, and delivery of Seniors Health & Wellness programming, through direct service delivery and/or the engagement and coordination of volunteers and student field placements, who assist with the delivery of services in seniors’ buildings and public spaces throughout Caledon.

The Manager also provides administrative oversight and direction to the Seniors Helping Seniors Program.  Seniors Helping Seniors is a self-directed program, within which seniors design and deliver services to other seniors, independently.

Health & Wellness Services are delivered through a collaborative team model with the goals of increased independence, improved health, wellness, and overall quality of life.

 

Position Deliverables/Accountabilities: Under the general direction of the Senior Manager, Health Services the incumbent.

•       Coordinate ongoing community based Senior Health & Wellness program implementation and development of new and additional programs

•       Plan, organize and facilitate social, recreational, educational, health and wellness and exercise-based programming in multiple Peel Living buildings and in public spaces throughout Caledon, including evaluating program delivery, effectiveness, and viability

•       Research and develop best practices for program delivery, health and wellness promotion and client centred service

•       Perform needs assessment to identify service needs and gaps and ensure existing and new program development ideas will meet these needs

•       Expand awareness of programs through networking and marketing materials

•       Develops volunteer position descriptions and works with CCS Volunteer Resources Specialist to recruit volunteers to assist with and/or deliver programs

•       Manages volunteers and academic field placement students to ensure the effective utilization of volunteers/students and provide support and mentorship

•       Recruit Seniors-Helping-Seniors (SHS) leads

•       Provides oversight, supervision, support, and direction to the Seniors Helping Seniors Program (SHS) Team Leads through the coordination and approval of an annual activity plan;

•       Maintain, develop, and cultivate strong working relationships with community partner agencies, related groups, CCS colleagues and clients

•       Tracks successes and measurable outcomes and utilizes this data to support future planning and the development of goals

•       Tracks and monitor expenses against a pre-set budget

•       Maintains detailed client files in strict compliance with confidentiality protocols

•       Works with CCS staff to ensure timely and appropriate referral of at-risk individuals;

•       Works collaboratively with community professionals

•       Behaves in a manner consistent with the values and ethics of CCS, appropriately representing the Agency in the community

Education

•       Post-Secondary education in a relevant field, such as Activation/Gerontology; Therapeutic Recreation; Recreation and Leisure

•       Certification in a health-related field with a combination of experience in service delivery to seniors

 

Nature of Experience Required

•       3 to 5 years of experience designing and delivering therapeutic recreation/leisure, activation, or health and wellness activities programming for seniors, vulnerable person, and persons with disabilities.

•       Experience leading & coordinating employees/volunteers/field placement students in direct service delivery.

Knowledge Required

•       Community based in-home health services.

•       Health promotion and community development

Skills Required

•       Leadership, facilitation, and planning

•       Excellent communication skills written and verbal.

•       Strong interpersonal skills

•       Demonstrated ability to work effectively independently and in a team environment.

•       Proficient in MS Office 365 (web)

•       Knowledge of InterRAI and Goldcare

Other Requirements

•       Access to a personal device (smart phone or tablet) is required for periodic, two-factor authentication using the Microsoft Authenticator App when signing into CCS networks/cloud environment as part of I.T. security protocols

•       Valid Driver’s license, reliable access to a vehicle & insurance

•       Clean Criminal Records Check – Vulnerable Sector

•       Full Covid-19 Vaccination (based on most recent Vaccination Policy & subject to change pending future policy requirements for this position)
