Wages: $25.47/hour for 21 hours/week
This is a bargaining unit position.

Scope
We are seeking a highly motivated and adaptable Seniors Active Living Facilitator to join our dynamic team. The senior’s active living facilitator operates in a high-demand environment, requiring exceptional organizational skills, the ability to prioritize effectively, and a proactive approach to problem-solving.
The Seniors Active Living Facilitator is responsible for developing and delivering program activities, including wellness and social activities, targeting seniors from local ethnic groups of low-income and isolated seniors. The facilitator will engage community partners to implement recreational, educational, health, and social events. The facilitator is also responsible for recruiting senior participants, reaching out to house-bound and vulnerable seniors, providing individual needs assessments, connecting seniors to resources, and conducting regular follow-ups.
