George House offers bed-based housing for youth aged 14-16, providing essential support for high-needs youth in care. Embracing a harm reduction philosophy, the program fosters collaboration with internal and external partners to ensure coordinated, integrated, and high-quality services.

 

As the Intensive Case Coordinator you will: 

  • Intervene and manage client behaviours by using Crisis Intervention Training (CIT) techniques.
  • Utilize the Agency’s Client Service Model, facilitate development of strength-based service plans with clients and provide support in working on the plan with clients.
  • Support the Plan of Care developed in consultation with the clinical lead.
  • Advocate for clients when necessary and assist clients in assertiveness and in developing advocacy skills for themselves. 
  • Provide individual and/or group life skills support to clients using the cognitive behavioural approach to teach skills such as problem-solving, anger management, self-control and coping strategies.
  • Communicate with parents, guardians, and referral sources by sharing information, addressing concerns, and resolving problems.

Hours of work: 37.5 hours a week, Monday to Friday, 10:00 am to 5:30 pm, including one late-night per week, with flexibility to accommodate program needs

 

COMPENSATION AND BENEFITS

Annual salary: Starting at $52,787.00

REQUIREMENTS AND QUALIFICATIONS

•         College Diploma in Social Services preferred

•         Current registration in good standing with a governing body such as the Ontario College of Social Workers and Social Service Workers (OCSW/SSW), the College of Registered Psychotherapists of Ontario (CRPO) etc.

•         Minimum 2 years’ combined student placement and/or paid experience in a mental health and/or substance use clinical or group setting

•         Demonstrated ability to apply relevant theoretical models when providing assessments to identify strengths, needs and level of risk

•         Current and demonstrated knowledge of integrated services to meet unique needs of individuals with complex needs, and youth at risk

•         Must have a valid Driver’s License

•         Must have access to reliable transportation, as some travel may be required

 

Start date:  April 28, 2025; as George House is a new program, all new hires are expected to participate in agency and program onboarding from April 28-May 9, 2025
