Parkdale Queen West Community Health Centre (Parkdale Queen West CHC) is a dynamic, community-based health care organization serving mid-west downtown Toronto. General Responsibilities:
Reporting to the Manager of Clinical and Allied Health, this position is responsible for providing holistic health care to a full roster of clients, including those who live in our catchment area and those from our priority populations: individuals experiencing homelessness, people who use substances, individuals identifying as LGBTQ, individuals identifying as Indigenous, individuals who have low income, youth, and seniors. In addition to seeing clients in the clinic, the NP may provide primary care services to clients at outreach locations. The NP also acts as a supervisor to health care students and participates in research and quality improvement activities.
Our clinical team works closely with our mental health, harm reduction and health promotion teams to ensure cross-team and cross-site collaboration and the NP will work with staff from those teams in providing service to individual clients, as well as in program development and implementation.
Skills & Qualifications:
1. Current registration as a Registered Nurse (EC) with the College of Nurses of Ontario (CNO), RNAO membership is an asset
2. 3 to 5 years nursing experience in a community setting, public health setting, and/or primary care
3. Knowledge and experience in mental health, harm reduction and chronic illness is required
4. Strong oral and written communication skills
5. Strong problem solving and adaptive skills
6. Excellent decision-making skills
7. Understanding of anti-oppression principles and frameworks
8. High level of comfort with diverse populations; demonstrated ability and comfort in working with unsheltered populations, people who use drugs and people with mental health challenges
9. Able to work independently and as a team member within a multi-disciplinary environment
10. Second language is an asset
Compensation: $115,525 to $137,500 per annum, plus Group Benefits and HOOPP (Health Care of Ontario Pension Plan)
