General Job Description:
The Transitional Case Manager will support the Mobile Crisis Rapid Response Team which includes the Integrated Response Worker as well as the Safe Bed Program in partnership with uniformed OPP officers.
Qualifications:
A regulated health professional preferred, experience working with community partners and an awareness of the local service system.
Job Status: Full time permanent.
Salary: per Collective Agreement

Responsibilities:
- The Core Activities of the Transitional Case Manager is outreach and client identification; assessment and planning; direct service provision/intervention; and information, liaison, advocacy, consultation, and collaboration.
This position is intended to:
- Increase capacity for Mobile Crisis services that involve police-partnered response teams, which respond to people in mental health and addictions crisis, to support persons with mental disorders/mental illness and minimize their interaction with the criminal justice system.
- Support individuals in crisis with referral to a mental health and addictions worker on emergency service calls with police.
- Facilitate access to designated safe beds when available and appropriate; and support individuals in crisis with referrals to appropriate, community-based mental health and addictions services.
The Transitional Case Manager on this team will:
- Have well developed motivational-interviewing skills to capitalize on client’s motivation.
- Have strong knowledge of and utilize trauma-informed care: understanding the impacts of physical, psychological, and emotional trauma and educating others to support effective strategies for supporting those affected by (multiple) trauma(s).
- Employ a psychosocial rehabilitation approach to care with a recovery focus.
- Have the ability to develop strong partnerships and/or service agreements with other agencies, community services, police, hospitals, or primary care providers to ensure continuity of service provision, as required (e.g., addictions services, employment training, developmental services, family support, etc.)
- Work in an outreach capacity, which may include supporting individuals virtually.
- Take an active role in the Regional MCRRT Community of Practice.
- Work closely with the MCRRT team and other community/justice partners to identify and engage eligible service recipients. Transition planning should start at the outset of service provision by identifying the clients immediate and longer-term needs and engaging with service providers as appropriate.
- Follow a Harm Reduction approach to treatment. Harm Reduction is an evidenced-based, client centered approach that seeks to reduce the health and social harms associated with addiction and substance use, without requiring people to abstain from or stop the use of substance.
- Ensure that all Policies and Procedures reflect a Harm Reduction approach to treatment.
- Accept clients on Opiate Agonist Therapy (OAT), or anti-craving medication (i.e.: Naltrexone, Acamprosate, etc.) that is supporting the client to reduce the harms associated with addiction and substance use.
- Develop and maintain strong pathways with all relevant community stakeholders (police, hospital, community addiction and mental health providers, primary care, etc.).
- Be open to working with clients living outside of the city but within the Sub-region. These clients may be supported via virtual technology and/or telephone where internet connectivity and/or telephone capabilities permit, and where it is medically safe to do so.
- Perform any and all other duties that may be required from time to time as requested by the Executive Director.
Ability to commute/relocate to Elliot Lake required.
