On behalf of the West Toronto Ontario Health Team (WTOHT), LAMP Community Health Centre is seeking a Project Manager for the Locally Driven Population Health Models (LDPHM) initiative.

LAMP Community Health Centre is a multi-servi657ce, community based charitable organization that provides a wide range of programs and services to improve a person's health. LAMP uses a co-ordinated approach to connect people to medical services and other programs that build personal resources. By providing services that improve physical, emotional, social and economic well-being, people can better control their health and environment.

Job Summary

We are seeking a Project Manager to oversee the implementation and coordination of the Locally Driven Population Health Models (LDPHM) for the West Toronto Ontario Health Team (WTOHT). Led by LAMP CHC, this project will focus on areas within the WTOHT catchment. The role involves collaborating with OHT partners, patients, caregivers, community organizations, grassroots groups, and faith-based organizations to promote preventative care, health promotion, and access to primary healthcare. The Project Manager will ensure service delivery aligns with objectives and meets funder accountability requirements.

The successful candidate will lead the development and maintenance of interdisciplinary teams, including clinical providers, community health workers, and preventative care ambassadors across West Toronto. This position will report directly to the Senior Director, Community Programs at LAMP CHC, with direction supported by the WTOHT.

Primary Responsibilities

• Provide leadership and operational management to an interdisciplinary team, ensuring alignment with LAMP’s values, mission, vision, and strategic objectives.

• Oversee the implementation of the Locally Driven Population Health Models (LDPHM) in collaboration with the WTOHT and other partners.

• Promote community engagement, capacity building, health literacy, and service access by prioritizing facilitation of care and offering broader wellness and prevention support.

• Support efforts to increase attachment to primary care and enhance participation in preventative screenings, such as the Ontario Breast Screening Program.

• Monitor programs and services continuously, identifying risks and driving ongoing improvements.

• Collaborate with internal and external stakeholders to enable integrated projects and foster partnerships.

• Manage program data collection, monitoring, evaluation, and funder reporting.

• Organize and support engagement activities and promotional events, including outreach, networking, capacity building, volunteer engagement, and facilitation.

• Assist in executing large-scale community events and coordinating attendance at relevant activities.

• Contribute to health-related messaging and promotion via social media and other platforms.

• Proactively manage project scope changes, ensuring responsiveness to evolving community needs.

• Perform additional duties as required within the project scope.

Required Qualifications and Skills

• University Degree in Health Sciences, Management or recognized equivalent work experience (Master’s degree strongly preferred)

• 3-5 years of project management experience in a health care environment, community-based environments an asset

• Regulated Health Professional designation is an asset

• Proven experience working independently and in partnership with a high level of proficiency in partnership management

• Demonstrated understanding of vaccine hesitancy and potential barriers to accessing vaccine and health promotion programs and services by residents in the community

• Good knowledge of health promotion, health education and health issues affecting newcomers, low income, multi-lingual and racialized communities

• Experience collaborating with other community organizations and working in partnership with residents and community ambassadors

• Experience in event and program planning, coordination and outcome evaluation

• Working knowledge of the current Ontario health care system is an asset to provide contextual understanding to OHTs and their potential for system transformation

• Sensitive to working with people of diverse cultural, age and economic backgrounds as well as marginalized individuals

• Excellent interpersonal and communication skills both written and verbal, with individuals and groups

• Demonstrated commitment to working from an anti-oppression perspective and creating pathways of access for equity deserving groups

• Demonstrated problem-solving, organizational and time management skills

• Able to travel within the GTA and to work flexible shifts including evenings and occasional weekends

Proficiency in the following computer skills

• Experience and ability to work effectively independently and remotely as part of a team

• Strong time management, self-direction, and other professional skills

• Access to home office with updated computer, software, Wi-Fi and data, audio/video capability

• Knowledge and use of tele and video-conferencing software, including Zoom, WebEx, and GoToMeeting, and Microsoft Teams

• Experience and ability to work within a multi-ethnic, multidisciplinary and multi-sectoral setting

• Excellent oral and writing skills knowledge

• Excellent computer skills, including ability to use Publisher, PowerPoint, Excel, Word, designer software and social media platforms
