Salary:

  • Registered Practical Nurse (RPN): $26.77 to $31.92 per hour
  • Registered Nurse (RN): $37.74 to $44.53 per hour
  • Nurse Practitioner (NP): $61.75 to $73.37 per hour

Job Summary

We offer a variety of nursing roles that allow for both professional growth and meaningful impact, including

Primary Care Nursing: Work along side physicians, allied health providers and community supports to provide comprehensive care to individuals and families across the lifespan.

Nurse Educator – Diabetes Education Program: Engage in client and group education to support diabetes management and prevention, collaborating with dietitians, other allied health professionals and client primary care providers.

Community HUB Nursing – BAC (Black African Caribbean) Health Focus: Join a new and evolving model that responds directly to the unique health and wellness needs of BAC communities through integrated, culturally safe and trauma-informed care.

Refugee Health Program: Contribute to a welcoming and responsive care model for newcomers and refugees, helping to reduce barriers and improve access to care.

Population Health and Intradisciplinary Teams: Be part of initiatives that partner with iHelp Centre, Toronto Community Housing and West Toronto Ontario Health Team with insight and local input to identify and address health disparities within the community.

Position Specific Responsibilities & Roles Available

NP Primary Care: Assesses, diagnoses and manages acute and chronic conditions independently, while contributing to preventative care and health promotion in a collaborative family practice environment. Several locations available.

  • Primary Health Team (main) 1 full time permanent position available (1.0 FTE)
  • Refugee Health Program - 1 full time contract position available (1.0 FTE)
  • Population Health 1 full time contract position available (1.0 FTE)
  • BAC Community HUB 2 full time permanent positions available (1.0 FTE each)

RN Primary Care: Provides comprehensive nursing care, clinical intake support, ordering and inventory control, health education and chronic disease management within a team-based, community family practice setting.

  • Refugee Health Program – 1 part time contract position available (0.5 FTE)
  • BAC Community HUB – 3 full time permanent positions available (1.0 FTE each)
  • Population Health 1 full time contract position available (1.0 FTE)
  • Primary Health Team (main) 1 full time permanent position available (1.0 FTE)

RN Nurse Educator – Diabetes Education Program: Promotes healthy community living by supporting diabetes self-management for clients 18+ living with or at risk of Type 2 Diabetes. Our teams consist of Registered Nurses, Dietitians and some Chiropody resources and offer service through one-on-one counselling, group sessions, advocacy and outreach at multiple locations.

  • West Toronto Diabetes Education Program 1 part time contract position (0.6 FTE), 3 full time permanent positions (1.0 FTE each), 1 part time permanent position (0.6 FTE)
  • BAC Community HUB – 1 full time permanent position available (1.0 FTE)

RPN Primary Care: Provides routine nursing care, supports health monitoring and education, ordering and inventory control, and assists with clinical procedures under the guidance of RNs, NPs, and Physicians within a team-based, community family practice setting.

  • Primary Health Team (main) – 1 full time permanent position available (1.0 FTE)

Required Qualifications

NP Primary Care:

1. Master’s degree in Nursing, or a combination of education and experience as sanctioned by the College of Nurses of Ontario;

2. Current and valid registration in good standing with the College of Nurses of Ontario as a Registered Nurse, Extended Class.

RN Primary Care:

1. Bachelor of Science in Nursing (BScN) and in good standing with the College of Nurses of Ontario (CNO) as an RN;

RN Nurse Educator:

1. Bachelor of Science in Nursing (BScN) and in good standing with the College of Nurses of Ontario (CNO) as an RN;

2. Certified Diabetes Educator (CDE) credentials or current enrollment would be an asset.

RPN Primary Care:

1. Current registration and in good standing with the College of Nurses of Ontario (CNO) as an RPN;

All positions:

1. Strong interpersonal skills with the ability to support marginalized populations facing complex health and social challenges, including those impacted by discrimination.

2. Experience in primary care or community health preferred.

3. Excellent communication skills, including the ability to communicate information and ideas to support understanding with diverse demographics, newcomers, and individuals with language barriers

4. Demonstrated commitment and understanding of primary health care, harm reduction, health promotion, Anti-Oppression; Harm Reduction, community-based practice, and the social determinants of health.

5. Familiarity with Ontario’s health system, and knowledge of immigration and health policy

6. A valid driver’s license is considered an asset

7. Knowledge of local communities, priority populations, or fluency in a second language reflective of the community served is an asset.

8. First Aid and BLS certification required and must be kept current.
