Job Title: Recovery Case Navigator

Based at: Herb Jamieson Center (10014-105A Ave NW),

Reports to: Rapid Response Team Coordinator

Hours of work: Monday to Friday, 9:00AM to 5:00PM.

Job Summary: Provide immediate and follow up care following medical emergencies, support clients with referrals to appropriate programs, including outside and inside agencies, develop recovery plans with clients and build lasting connections with recurring clients.

Minimum Qualifications and Skills:

  • Recovery Coach Certification or ability to complete this training
  • Lived Experience/ Exposure
  • Strong interpersonal skills
  • Sign and adhere to the Hope Mission Statement of Faith

Key Responsibilities:

  • Support and facilitate case management and care with other recovery coaches on site. 
  • Work with participants to develop recovery plans which are person centred and strengths based.
  • Resource navigator – identify appropriate recovery resources internal and external to Hope Mission and support the client by advocating for them and providing education on resources available to them.
  • Referral management – refer clients to appropriate resources and follow up with the outcomes of the referral.
  • Provide follow-up to build rapport and ensure continued support.
  • Maintain documentation, including referrals, service delivery, and participant records in accordance with FOIP.
  • **Uphold and live out the Statement of Faith**

Other Requirements:

  • Being a Christian Organization, we require our employees to be in agreement and sign the Hope Mission Statement of Faith.
  • Must be legally entitled to work in Canada on a full-time basis.
  • Able to provide Criminal Record Checks and Vulnerable Sector Search
  • A letter of reference from a ministry or church is an asset
  • Adherence to Hope Mission’s Policies and Procedures.
