Job Summary

Position Specific & General Responsibilities
NP Refugee Health Program: provide culturally responsive and client-centered interventions to newcomer and refugee populations. This includes testing and treatment for infectious diseases, preventative screening, perinatal, sexual health and family planning care, pediatric care and making connections to longitudinal primary care and other resources as required to meet needs.
NP Population Health: Provide health prevention screening, risk assessments, diagnosis, treatment and referrals to specialist/allied/social services care where appropriate, to underserved community members facing barriers to accessing health care. Team-based care is done in high priority neighbourhoods in West Toronto with partner organizations through iHelp Centres and other pop-up locations.
NP Primary Care: Provide comprehensive, client-centered primary care across the lifespan, working independently and collaboratively within an intradisciplinary team. Participate in communities of practice including gender affirming care and maintain ongoing development relative to priority populations and equity driven initiatives.
All Roles:
Provide primary health care services including assessment, diagnosis, treatment, prescribing and management of acute and chronic conditions, in accordance with the defined Primary Health Care Nurse Practitioner extended class scope of practice, Bill 179 (Regulated Health Professions Statute Law Amendment Act, 2009) and the Controlled Drug and Substances Act (CDSA).
Maintain detailed and accurate electronic medical records (EMRs) in accordance with professional and organizational standards; participate in chart audits, peer reviews and ongoing quality assurance and improvement initiatives.
Contribute to the development and refinement of clinical workflows, protocols and procedures.
Maintain clinical competence by staying current with evidence-informed best practices, regulatory and professional requirements (including those set by the College of Nurses of Ontario), and by engaging in ongoing learning related to cultural safety, trauma-informed care, the social determinants of health, and health equity to support clients served by the CHC.
Required Qualifications
Master’s degree in Nursing, or a combination of education (BScN + NP graduate program) and experience as sanctioned by the College of Nurses of Ontario;
Current and valid registration in good standing with the College of Nurses of Ontario as a Registered Nurse, Extended Class.
Strong interpersonal skills with the ability to support marginalized populations facing complex health and social challenges, including those impacted by discrimination.
Experience in primary care or community health preferred.
Excellent communication skills, including the ability to communicate information and ideas to support understanding with diverse demographics, newcomers, and individuals with language barriers
Demonstrated commitment and understanding of primary health care, harm reduction, health promotion, Anti-Oppression; Harm Reduction, community-based practice, and the social determinants of health.
Familiarity with Ontario’s health system, and knowledge of immigration and health policy
A valid driver’s license is considered an asset
Knowledge of local communities, priority populations, or fluency in a second language reflective of the community served is an asset.
First Aid and BLS certification required and must be kept current.
