Job Summary
Under the supervision of the Manager of Health Promotion & Community Relations and working within the objectives of the program, the Community Health Worker (focus on Newcomers, Immigrants & Refugees) will lead the delivery of support services for priority populations and advocate for greater action on community health issues in Peel. The Community Health Worker is responsible for developing and implementing ongoing community mental health, wellness and social support programs for Newcomers, Immigrants and Refugees at LAMP’s East Mississauga site and ensure that all organizational policies are followed.
This position will be responsible for using the CHC Model and Social Determinants of Health framework in order to ensure services are accessible. This includes supporting internal and external client referrals, sharing resources, advocacy on behalf of clients, and participation in individual case coordination as appropriate. This position will also work with an interdisciplinary team and with community partners to effectively and efficiently serve clients, while practice innovative approaches to service delivery applying a health equity lens.
Primary Responsibilities
Plan, implement and evaluate community programs with a focus on promoting physical health, mental health and wellness and social engagement in accordance with workplan.
Lead and/or co-facilitate group programs and workshops, both in-person in the East Mississauga community, as well as virtually online.
Advocate on behalf of clients and participate in case coordination in cooperation with EMCHC’s interdisciplinary teams and partners, to ensure those with complex needs from our priority populations have access to information, referrals, follow-up and system navigation support.
Actively conduct outreach to clients and communities by organizing/attending community events, sharing information on online platforms, and promoting East Mississauga and LAMP programs.
Support the development and distribution of health promotion and communications resources/materials to clients and communities.
Engages with stakeholders including funders, partners, community members to assess changing needs of Newcomers, Immigrants, and Refugees, tailor programs accordingly and measure outcomes.
Support and mentor students and volunteers who may support programs as needed
Administrative duties, including: registration and attendance, preparing invoices, data entry etc.
Engages in ongoing conflict resolution and mediation with clients.
Works from a strengths-based client centred approach to assist in building client’s own capacity to achieve health and well-being.
Support Newcomers, Immigrants and Refugees to identify barriers to services, organize community activities and events, advocate for the good health of the community and perform other tasks, as appropriate.
Provide intake assessment support for the range of services offered at EMCHC, using eligibility criteria.
Actively conduct outreach to clients and communities by organizing/attending community events, promoting our programs and working on our promotional materials for health promotion programs and services.
Develop new partnership and foster existing partnerships with organizations for greater collaboration.
Required Qualifications
Undergraduate degree in a Health or Social Science discipline or significant equivalent in health promotion and/or community development
Three to five years relevant experience in a diverse community setting, using an anti-oppression and anti-racist approach to community development.
Previous experience working with Newcomers, Immigrants & Refugees and/or Black, Indigenous, People of Colour and Queer & Trans (BIPOCQT) communities.
Experience in effective community outreach practices using community development models for diverse
racialized populations with intersectional identities.
Strong group facilitation skills and communication skills with a diverse audience.
Positive attitude, empathetic, flexible and dependable with strong initiative and the ability to work both independently and in a team-oriented atmosphere.
Able to handle crisis situations and deescalate in a supportive manner.
Excellent interpersonal skills necessary to work effectively across all levels of the organization’s workforce.
Excellent communication skills, both verbal and written, with the ability to clearly convey information & ideas.
Excellent knowledge of community resources and social support programs in Peel for Newcomers, Immigrants and Refugees, Able to develop and foster relationships with partner agencies in order to provide the best services to clients.
Knowledge of East Mississauga communities, Peel Region organizations and services, and/or a second language that is reflective of the community being served are all.
