Reports to: Program Manager
Hours: 48 hours/week (12-hour shift, 4 days on – 4 days off; 8 am – 8 pm)

Job Summary

The Intake/Admission Worker serves as the first point of contact for clients entering the Wellspring Program. This role is focused on building trust, conducting thorough assessments, facilitating admissions, and enforcing house policies to ensure a safe and supportive environment.

Key Responsibilities

Team Collaboration

  • Participate in regular at-staff debriefings with Wellspring staff.
  • Provide accurate and timely updates to both clients and team members.
  • Coordinate with aftercare facilities and other external partners to problem-solve and support clients’ transitions.

Medication Management

  • Secure and maintain the medication cabinet and related documentation.
  • Coordinate medication orders (including PRNs) with pharmacies and ensure proper documentation.
  • Administer medications to clients as required in accordance with policy.

Safety & Crisis Intervention

  • Monitor client rooms and common areas to ensure safety and adherence to house rules.
  • Conduct random drug and alcohol screenings as required.
  • Maintain strict confidentiality and ensure clear communication with staff and clients.
  • Employ non-violent crisis intervention and provide first aid when necessary.

Admissions

  • Distribute application packages and respond to requests for them.
  • Review and process applications, interview prospective clients, assess eligibility, and refer for medical or psychiatric screening as needed.
  • Coordinate referrals with shelters, physicians, and external agencies and manage networking/partnerships.
  • Arrange transportation, and coordinate with the pharmacy for medication communication.

Client Intake/Discharge

  • Welcome and orient new clients to the program and facility.
  • Conduct urine/breathalyzer tests and perform bag checks as part of the intake process.
  • Enter client data into the electronic system (Efforts to Outcome – ETO) and update the bed board.
  • Complete required paperwork and follow up on any missing information.
  • Client Discharge: Carry out the discharge process for clients leaving the program, which includes arranging transportation, packing and managing medications, coordinating with the pharmacy for any discharge‐medication communication, liaising with the Wellspring Admissions Team, and ensuring all exit paperwork and referrals are completed.

Housing Coordination

  • Document any housing-related incidents and support safe transitions within housing policies.

Recovery Support

  • Develop rapport with clients through active listening, counselling, and (where appropriate) prayer.
  • Support program delivery and facilitate or assist with recovery-related classes and sessions.

Administration

  • Supervise client chores and enforce adherence to house rules.
  • Enter admission and intake data into ETO and maintain accurate client records.
  • Monitor emails and ensure you are aware of program proceedings and updates (e.g., team/house meetings).
  • Update the intake office calendar and ensure any unfinished tasks from the previous shift are completed (e.g., paperwork, food disposal, storage organization).
  • Shred outdated documents, upload monthly MAR sheets to ETO, and file physical copies.
  • Review client files for missing signatures/forms; organize and notify day staff of any issues via email.
  • Prepare occurrence reports (OR) when necessary (e.g., hospital admissions, relapses, unusual activities). Discharge clients in ETO when needed (e.g., relapse, departure, hospital admission).

Other Tasks

  • Pack and clean client rooms following discharge or program exit.
  • Clean and sanitize the intake office.
  • Organize pantry cabinets, case-management room shelves, storage areas, the laundry room cabinet, and the electrical room.

Requirements

Education

  • Degree in Social Sciences, Nursing, Pastoral Studies, or a related field.

Skills & Qualifications

  • Strong analytical judgement and decision-making abilities.
  • Excellent written and verbal communication skills.
  • Strong interpersonal skills with the ability to engage effectively with diverse populations.
  • Ability to lead, mentor, and support a small team when required.
  • Experience with the ETO system is an asset.
  • Proficiency in Microsoft Office (Word, Excel, Outlook etc.).
