Position Title: Case Manager – Wellspring Pre-program
Reports To: Program Manager
Hours of Work: 40 hours per week

Job Summary
The Case Manager for the Wellspring Program supports marginalized women on their journey of healing from trauma and addiction through a holistic, Christian-based approach. In this role you will facilitate classes, provide individualized support for clients, and coordinate case management services that foster recovery and growth.
Major Duties & Responsibilities
Program Participation & Spiritual Leadership
- Support women dealing with addiction, mental health challenges, and trauma.
- Build relationships that focus on the heart, not just the behaviour; address issues arising within the program environment.
- Participate in spiritual leadership: prayer, team support, and devotion.
- Support team members and contribute to a collaborative, faith-based culture.
- Maintain weekly records of program sessions and case management activities.
Program Coordination & Facilitation
- Facilitate groups and classes with strong group facilitation skills.
- Participate in program development, implementation, evaluation, and revisions.
- Schedule guest speakers or volunteers when needed, and supervise volunteers or practicum students if applicable.
- Connect clients to mental health, housing options, and community advocates.
- Attend weekly meetings with Wellspring staff to review program delivery and client progress.
Case Management & Client Support
- Meet weekly with individual clients to help them identify and face fears, with the support of God and community.
- Employ constructive conflict-resolution and disciplinary processes as needed.
- Monitor client calendars, follow up on meetings and appointments, and assess individual recovery plans.
- Advocate for clients when they break building policies or need additional support.
- Demonstrate a non-judgmental attitude toward all clients and work with an understanding of the Stages of Change and Motivational Interviewing techniques.
- Guide clients through the Genesis Process tools and recovery framework.
Self-Care & Professional Development
- Meet monthly with the Program Manager or Coordinator to review workload and well-being.
- Adhere to the defined work hours and responsibilities.
- Debrief regularly with a designated support person and access support services as required.
- Maintain clear boundaries between staff responsibilities and resident responsibilities.
- Engage in ongoing learning: attend courses, network within the community, and stay current with relevant literature.
Additional Responsibilities
- Report relapse or occurrence feedback to the Manager.
- Ensure participants on your caseload maintain good community standing (e.g., room tidiness).
- Perform other duties and special projects as assigned by the Manager.
- Must be able to work independently and in group settings under pressure, while maintaining a flexible schedule.
- Willingness to assist with or volunteer for fundraisers.
- As a Christian organization, employees must agree with and sign the Hope Mission Statement of Faith.
- Must be legally eligible to work in Canada full-time, provide a criminal record check including the vulnerable sector, and adhere to all Hope Mission policies and procedures.
Educational Requirements
- Degree in Social Sciences, Nursing, Pastoral Studies, or a related field.
Minimum Qualifications & Skills
- Agreement to undergo Genesis Process training.
- Strong analytical skills and sound decision-making ability.
- Excellent written and verbal communication skills.
- Strong interpersonal skills and ability to engage effectively with diverse populations.
- Able to lead, mentor, and support a small team.
- Experience with ETO (Efforts to Outcome) is an asset.
- Intermediate proficiency in Microsoft Office.
- Experience in addiction and mental health services.
- Valid Class 5 driver’s licence.
