This permanent full-time average 32/wk position provides support to Topaz Place, First Avenue and the Managers of Care by overseeing assessments, care, recovery planning, medication administration, and treatments, while using nursing judgment in routine and emergency situations. It provides leadership to all Health Care Workers (HCWs) and Licensed Practical Nurses (LPNs) to ensure high standards of care for persons served. Responsibilities may include 24/7 on-call availability, with duties performed in line with policies that respect the dignity, autonomy, and independence of the persons served.
SPECIFIC DUTIES
- Oversees development and monitoring of health care/Recovery-Centered Clinical System plans (RCCS) and goals for the persons served at Topaz Place/First Avenue; addressing the physical, psychological, social, and spiritual needs of each. Regularly communicates with MOC to provide updates and ensure the plans align with the needs and goals of the persons served
- Meets with Mental Health Case Manager and MOC quarterly to review and monitor care/recovery plans and ensures recovery plans are maintained
- Provides direct nursing care, including administration of medications, medical treatments and documented procedures as per provincial regulations and facility policies and procedures and ensures medication administration compliance. Works closely with pharmacist/pharmacy to assess and promote the recovery journey for persons served
- Using good body mechanics and techniques; lifts, transfers and ambulates persons served in a safe manner
- Provides leadership/training to other staff in these areas as necessary
- Uses established infection control techniques to prevent and control the spread of infection and provides ongoing teaching with staff and persons served. Attends related in-services as required
- Assists with the admission, transfer, and discharge of persons served
- Evaluates the effectiveness of nursing care plans and treatments, and informs the MOC and/or the physician of any significant changes in the condition of the persons served
- Assists the physician when necessary; receives and carries out doctor’s orders, setting up medical appointments and maintains a good rapport with physicians and other medical practitioners (psychiatrist, case manager, mobile lab services, dentist, etc.)
- Maintains records of the persons served to the required standards: updating the medical needs, oral/nutritional care plans and metabolic monitoring, etc. of the persons served
- Responsible to ensure nursing equipment and designated work areas are kept clean and tidy
- Responsible to ensure that supplies/medications of the persons served are ordered, received and stored
- Attends meetings/in-services as required and facilitates medical and psychiatric concerns at staff meetings
- Establishes and maintains good interpersonal relations with persons served, staff, visitors and volunteers
- Together with the MOC, addresses work performance issues related to the care of the persons served
- Assists MOC with administrative tasks as needed
- Other duties as required
REQUIREMENTS
- Current registration with the BC College of Nurses and Midwives (BCCNM)
- Adherence to the Standards of Practice in B.C. set by BCCNM
- Recent experience in related field is an asset
