Scope of the Position:

The Alberta Brain Injury Network Program Supports for Community Living worker is part of a larger provincial network providing service coordination of short-term supports to individuals and families living with the impact of an acquired brain injury. Working from a trauma-informed perspective, the Supports for Community Living worker will promote a person-centered, rehabilitation approach to service delivery, respecting each client’s rights as an individual. Supports will be provided according to principles and philosophy of the Canadian Mental Health Association.

Key Responsibility: Support to Clients

• The Supports for Community Living worker will work with a Service Coordinator to identify specific, short-term goals for between 10-15 individuals.

• The Supports for Community Living worker will respect and protect the culture, service users, including but not limited to their right to independence, autonomy, and self-determination; their right to choose and practice individual values, beliefs, religion, and culture; and their right to privacy and dignity.

• The Supports for Community Living worker will work with individuals in their own homes and in the community to improve their abilities and to develop capacity for independent living.

• The Supports for Community Living worker will assist individuals to set future goals and identify achievable steps towards those goals to encourage success.

• The Supports for Community Living worker will receive mileage reimbursement only for travel from an approved designated starting location to the client’s residence/meeting location. The designated starting point will be identified and communicated at the time of assignment.

• The Supports for Community Living worker will provide services within the designated region. Specific coverage areas will be determined based on client needs and organizational planning.

Key Responsibility: Reporting and Data Collection

• The Supports for Community Living worker will complete monthly reports.

• The Supports for Community Living worker will assist with satisfaction surveys and other data tracking information required on a monthly/quarterly/annual basis and will report this information to the appropriate Service Coordinator as required.

• The Support for Community Living worker will plan ahead regarding scheduling with clients to ensure timely delivery of service to each individual.

• The Supports for Community Living worker may claim printing costs only when using the approved provider (Staples Printing); expenses related to personal printers, ink, or supplies are excluded from reimbursement.

• The Support for Community Living worker will complete other administrative duties as assigned.

Key Responsibility: Community Outreach & Engagement

• The Support for Community Living worker will canvas their service area for potential new clients and/or community partnerships. This may include attending interagency meetings or other requests within the service area from the Program Supervisor.

Qualifications

The ABIN Supports for Community Living worker will have a diploma or degree in the human services field as well as a minimum of three years of experience that includes extensive knowledge in the area of brain injury and rehabilitation techniques.
