Confederation Court Community House is a neighbourhood-based nonprofit organization committed to supporting children, youth, and families in a low-income community. We provide responsive, equitable, and strength-based programs designed to reduce barriers, build resilience, and strengthen community connections.

The Wraparound-Service Worker provides individualized, strengths-based support to families accessing our food bank and family support programs. This role focuses on holistic, client-centered service coordination—helping individuals and families navigate complex systems, access resources, and achieve self-identified goals.
The Worker builds trusted relationships and works collaboratively with internal programs and external partners to reduce barriers related to housing, income security, food access, mental health, education, employment, and community integration.
• Provide one-on-one wraparound support to individuals and families.
• Conduct strength-based assessments and collaboratively develop action plans.
• Assist clients in setting achievable goals and monitor progress.
• Support referrals to community services (housing, healthcare, employment, income supports, legal aid, etc.).
• Advocate on behalf of clients when navigating complex systems.
• Work closely with Food Bank staff to identify clients who may benefit from enhanced support.
• Provide intake support and follow-up with families accessing emergency food services.
• Assist with crisis intervention and stabilization planning.
• Support applications for income supports (e.g., Ontario Works, ODSP, child benefits).
• Assist with housing searches, landlord communication, and housing stabilization.
• Help clients access childcare, education, employment training, and community programming.
• Maintain strong working relationships with partner agencies.
• Maintain accurate and confidential client files.
• Track service outcomes and report on impact indicators.
• Participate in case conferencing and team meetings.
• Support funding and evaluation reporting as required.
• Participate in community outreach initiatives.
• Promote awareness of available supports.
• Contribute to a welcoming, inclusive, and culturally responsive environment.
• Diploma or degree in Social Work, Social Service Worker, Community Development, or related field.
• Minimum 2 years’ experience in case management, family support, or community-based social services.
• Strong knowledge of Ottawa’s social service systems and community resources.
• Experience working with marginalized and low-income communities.
• Demonstrated ability to work from a trauma-informed, anti-oppressive, and strength-based lens.
• Strong interpersonal, communication, and documentation skills.
• Ability to work independently and as part of a collaborative team.
• Valid Vulnerable Sector Police Check.
