About the Community Connector Program

The Community Connector program is a social prescribing initiative funded by United Way British Columbia and operated by local community organizations.

The program supports older adults (55+) to improve their health and well-being by addressing non-medical barriers such as social isolation, housing instability, transportation challenges, food insecurity, and limited access to community supports.

Through personalized, strengths-based conversations and structured follow-up, Community Connectors help participants identify what matters most to them and connect to meaningful community-based services, activities, and resources.

The goal of the program is to help older adults age well in their communities, reduce avoidable strain on the health system, and strengthen local networks of support across Greater Victoria.

Position Summary

The Community Connector is a full-time, in-person role primarily focused on delivering the Community Connector social prescribing program to older adults in Greater Victoria.

This position holds an active participant caseload and is responsible for conducting in-depth intake conversations, co-developing personalized wellness plans, connecting participants to community-based resources, and providing structured follow-up and support.

In addition to direct program delivery, the Community Connector contributes to community networking, partnership development, light program administration, documentation, and evaluation activities.

This position is preventative in nature and not designed as a crisis response or emergency outreach role.

Purpose of the Role

To support older adults (55+) to age well in their communities by:

  • Addressing social determinants of health
  • Reducing social isolation and increasing belonging
  • Bridging healthcare and community-based supports
  • Co-creating strength-based, personalized wellness plans
  • Building and sustaining local partnerships that enhance participant access

Key Responsibilities

1. Participant Engagement & Program Delivery

  • Maintain an active caseload of Community Connector participants
  • Conduct comprehensive intake meetings using a strengths-based “What matters to you?” approach
  • Assess social determinants of health including isolation, housing, transportation, food security, financial stress, and community participation
  • Co-develop personalized wellness plans aligned with participant goals
  • Facilitate referrals, warm introductions, applications, and supported connections to community resources
  • Provide structured follow-up at established program intervals (e.g., 3, 6, 9 months)
  • Maintain participant confidentiality and obtain informed consent in alignment with program requirements
  • Identify when needs exceed program scope and appropriately redirect to other services

2. Community Networking & Development

  • Build and maintain relationships with senior-serving organizations, allied health professionals, and community groups
  • Participate in local planning tables, communities of practice, and networking meetings
  • Support ongoing asset mapping in Greater Victoria to identify emerging resources and service gaps
  • Contribute to strengthening referral pathways between healthcare and community systems

3. Program Administration & Documentation

  • Maintain accurate and timely participant documentation and reporting records
  • Contribute to data collection and evaluation processes required by United Way BC and program funders
  • Support program tracking, outcome measurement, and reporting
  • Assist in maintaining program materials (brochures, referral forms, participant resources)
  • Participate in team meetings, learning opportunities, and program development discussions

Qualifications & Experience

  • Post-secondary education in social services, community development, gerontology, public health, or a related field (or equivalent experience) welcomed
  • Experience working with older adults, particularly those experiencing isolation, frailty, or barriers to community participation
  • Demonstrated ability to conduct assessments and co-develop support plans in a person-centred framework
  • Experience building partnerships and working collaboratively across sectors
  • Strong organizational skills with the ability to manage an active caseload
  • Proficiency with digital documentation systems and standard office software

Compensation & Benefits

This position is unionized under the BCGEU Community Social Services Collective Agreement and is classified at Grid 31 (Step 2 minimum).

The base salary is expected to fall within the upper $50,000 to low $60,000 range annually, based on a 37.5-hour work week, in accordance with the collective agreement and depending on experience. The position also includes a comprehensive benefits package as outlined in the collective agreement.
