Program Description – Hope Health

Hope Health delivers a variety of health services to clients utilizing Hope Mission’s shelter services. Hope Health’s nurse-led clinics provide accessible frontline healthcare support for individuals experiencing homelessness, addiction, mental health challenges, and housing instability. The clinics offer low-barrier health assessments, nursing care, wound care, medication support, referrals, and connections to broader community and recovery services, helping clients address urgent health needs while building stability and trust.

Position Summary

This position functions as part of a diverse team within a homeless shelter and addictions and recovery setting. Clients present with various physical, mental, and social health concerns, as well as challenges with addictions. The Registered Nurse utilizes clinical expertise, critical thinking, problem-solving, and a health promotion approach to provide holistic care. You will advocate for clients, complete health teaching and promotion, and continually grow your knowledge of community resources to guide clients toward appropriate services that meet their whole health needs and goals.

Within the parameters of your license and scope of practice, you will safely provide hands-on care to clients and advise shelter leadership on evidence-based practices to promote the health and safety of workers and clients. You will intentionally build relationships and rapport with clients to assist them in identifying and achieving their physical, mental, emotional, social, and spiritual health goals. This is a mobile role — you will be able to provide services at Hope Mission’s various Edmonton-based shelters, recovery sites, and rescue vans as needed.

Key Responsibilities

Client Care & Relationship Building

• Connect daily with shelter staff and guests to build rapport and assess holistic health needs.
• Facilitate the development, implementation, and administration of individualized care plans in collaboration with clients and other care providers, monitoring and evaluating progress and outcomes.
• Provide basic assessment and intervention of injuries and ailments within scope of practice, including wound care.
• Respond to on-site emergencies and provide appropriate interventions, such as overdose responses.
• Provide education for chronic disease management and medications as needed.
• Provide naloxone education to clients who request it and to staff as needed.

Collaboration & Advocacy

• Establish and maintain effective liaison with Hope Mission staff, community resources, and other agencies and stakeholders to ensure coordinated services.
• Connect clients with community resources and advocate for their health needs.
• Work with leadership to develop strong policies to promote the health and wellbeing of clients and staff.
• Documentation & Professional Development
• Complete accurate, succinct, and thorough documentation to record care provided to clients.
• Continually grow in personal knowledge of current evidence-based practices in mental health, addictions, chronic disease management, wound care, and caring for vulnerable populations.

Qualifications

Education & Experience

• Licensed Registered Nurse in good standing and able to practice in the Province of Alberta.
• Experience in inner-city or community health settings is an asset but not a requirement.

Skills & Competencies

• Able to balance numerous demands and conflicting priorities; strong problem-solving skills.
• Able to work cooperatively within a multidisciplinary team.
• Demonstrates patience and understanding for those with addictions, mental health, and/or behavioural disorders.
• Sound understanding of trauma-informed care and meeting clients where they are.
• Excellent communication skills and strong computer proficiency.

Other Requirements

• As a Christian organization, Hope Mission requires employees to agree with and sign the Hope Mission Statement of Faith.
• Must be legally entitled to work in Canada on a full-time basis.
• Able to provide a Criminal Record Check with a Vulnerable Sector Search.
• A letter of reference from a ministry or church is an asset.
