REACH ( ) is offering a unique and exciting opportunity to interested registered nurses to be part of innovating access to primary care in BC!


You will be instrumental in delivering high quality care as part of an interdisciplinary team in the REACH Urgent & Primary Care Centre (UPCC). REACH UPCC will be a new service operating alongside our medical, dental, pharmacy and multi-cultural family services.


This new program aims to provide urgent primary care to any individual who needs it 7 days a week (with extended hours). While the emphasis is on urgent primary care, our vision of a REACH U&PCC strengthens our existing commitment to the social determinants of health as a foundation for patient care, integrating supports and interventions to address these within the urgent care setting.

We are looking to fill two RN roles: The Care Coordinator position will be responsible for coordinating care and managing follow-up for complex cases. This will involve referral to community care providers and facilitation of patient attachment to longitudinal care. The Care Coordinator will work with the patient and interdisciplinary team to determine the best referral pathway and follow-up approach.

The Triage role will be responsible for assessing patients and determining their need of medical assistance, and communicating with and managing patients in the waiting room.
